Root cause = ONE owner-decision (S124 model-tier flip: H8 all-inherit → worker-tier-pin opus). hmw.js + 14/14 frontmatter = UPDATED (source). Derived docs/memory = NOT updated (residual). jump_on_class_repeat=3 → view-residual-asym at 8 >> 3, BUT this is ONE flip's doc-sync debt in ONE audit, not a cross-audit recurrence (first audit = seeding tally, no prior count).
