Nội-dung áp của id này đã nằm TRỌN trong docs/governance/adap-reports/2026-06-18-Governance-harness-10-flat-refine-checklist-v2.md (S72) — báo-cáo đó tự-khai hai broadcast nguồn ngay ở :5. File này sinh @S149 (2026-07-25) chỉ để /adap-audit parse được per-id (format-LOCK .claude/commands/adap-report.md:14), KHÔNG kèm việc mới.

Vì sao cùng nội-dung: v2 là công-cụ tự-kiểm cho H9+H10; SE chạy nó ra 3 gap thật (A8 sleep-command thiếu · run-trace còn subfolder · hmw.js meta stale) rồi vá trong CÙNG đợt S72. Trạng-thái sau-vá đã ghi per-PART ở report gốc :15-19; viết lại = chép bảng lần hai.

4. tailored-gì + skip-gì-vì-sao

Tailored (FORM): báo-cáo đặt tên theo nội-dung việc (harness-10-flat-refine-checklist-v2) thay vì theo id broadcast ⇒ chính là lý do id này trượt khỏi mọi lần quét exact-id, và là lý do file pointer này tồn tại.
Function-floor GIỮ: self-verify đúng-nấc từng mục PART A/B/C + tách {executed-file ⟂ runtime ⟂ convention} + reverse-findings gửi ngược (:38-42, 4 khoản).
Skip: không skip mục nào của v2. PART A8 (sleep-compress command) trước đó SE thiếu → checklist chính là cái bắt ra, đã port (:15, :25) ⇒ đây là "bắt-rồi-vá", không phải skip.

5. honest-caveat

🔴 Chống double-count /adap-audit: file này + 2026-06-18-Governance-h10-flat-detector-refine.md (pointer) + 2026-06-18-Governance-harness-10-flat-refine-checklist-v2.md (report gốc) = 3 file, 1 đợt-adopt duy-nhất S72. Đếm đúng: 1 đợt, 2 id-broadcast, 3 file report.
Cẩn-thận nhầm với v1: 2026-06-18-Governance-checklist-harness-9-10.md (v1, S71) là file khác, id khác — v1 có report đầy-đủ riêng. v2 supersedes v1; đừng gộp/đè.
Caveat gốc còn hiệu-lực: 1/4 auditor Part A failed-no-StructuredOutput → em-main self-gate từ đĩa, đã disclose (:45); hmw.js flat mới node --check PARSE-OK, RUN-TRACE runtime cần restart CLI (:48); B3 self-correction path/count của report S71 (:47).
Sha chưa recompute (declared-read only). Nấc verified KHÔNG tự phong (§C5) — chờ /adap-audit.
